Health Insurance Requirement
˿Ƶ requires all students that meet at least one of the criteria listed below to annually submit proof of active health insurance (photo of the insurance card) that permits access and utilization of medical care in the Buffalo New York area OR enroll in ˿Ƶ’s student health insurance plan through United Healthcare. Students must be enrolled for the full academic year.
If a student fails to complete this process by the semester deadline the student will automatically be enrolled in ˿Ƶ’s student health insurance plan and the premium (annual cost) will be added to the student’s bill.
- Students living in residential housing (including but not limited to Campus Proper, Snyder Park and Collegiate Village)
- Students who have international student status
- Students participating in an intercollegiate sport
- Students participating in the clinical portion of their academic program
The health insurance verification process is part of becoming “move in ready” for residential students and part of the athletics pre-season clearance process. Students will be sent additional information on completing this process ahead of the semester deadline.
Students that submit proof of their health insurance card are “WAIVING” ˿Ƶ’s student health insurance plan. Health insurance coverage that is submitted must have an effective date that begins prior to the start of classes for the semester in which the student is enrolling. ˿Ƶ partners with insurance broker Haylor, Freyer & Coon to review and verify each student’s health insurance coverage.
Students are not permitted to move into the residence halls and/or practice or play an NCAA sport until their health insurance has been verified.
- Student athletes: In addition to completing the insurance verification process outlined above, ALL student athletes must also submit proof of insurance to the athletics department through the Spry software system. A notification will be sent by the Athletics Compliance office indicating when the Spry systems is available. New student-athletes should refer to the "New Student-Athlete Guide" for instructions.
International students are required to enroll in ˿Ƶ’s student health insurance plan or an approved plan through Gallagher as listed below. These plans are vetted and ensure that international students have essential* health insurance coverage while in the United States. Traveler’s insurance plans are not accepted by ˿Ƶ. Students MUST be enrolled for the full academic year: 8/01/YY - 7/31/YY.
Student athletes who receive athletic scholarship funds to cover health insurance will be required to enroll in the ˿Ƶ Student Health Insurance Plan.

International Student Athletes are required to enroll in an insurance plan that provides comprehensive coverage in the Buffalo area. Students can select from The ˿Ƶ Student Health Insurance Plan or the Gallagher Sports Basic or Gallagher Sports Plus.
International Student Athletes who do not enroll in one of these plans could be subjected to out-of-pocket costs and risk not being covered by ˿Ƶ's Secondary Sports Insurance.

- New enrollees: complete the "Purchase a New Plan" process by entering your date of birth, school name and student category: "International Student" (applicable for most students) or "International Student Athlete" to review sport plans. Please note: you may need to switch between student categories to review all approved plans and make an informed decision.
- Important notes:
- When reviewing plans, look at the deductible (preferred provider option): The amount of money that you must pay before your insurance plan will begin covering medical expenses. For example, if you select "preferred provider $500" you are responsible to pay $500 out of pocket (as you seek care) before insurance will start covering medical expenses. There will be at least 2 options for the deductible/preferred provider so please review to make an informed decision.
- ˿Ƶ does not have a student health center on campus. Therefore there are no costs "waived at the student health center." Students who need to see a provider can request transportation from Campus Safety to a local urgent care center within 1 mile of campus. Student athletes can also work with the athletic training team to connect with the team doctor for evaluation.

- Once you enroll, you will receive emails from Gallagher and United HealthCare. Please READ these emails, SAVE them and TAKE ACTION as outlined below:
- When you receive your Coverage Confirmation, send a copy to health@daemen.edu ASAP so the CHIP Center can verify your plan details.
- Student athletes must also submit a copy to the athletics department through the ARMS software system.
- Temporary ID Card: save a photo to your phone and/or print a copy to keep on you until you can access your digital ID card.
- Digital ID Card: you will NOT be sent a physical ID card. To access a digital ID card to have on a smart device (or print a copy to have on your person):

- Visit , login and download a copy of the card (available within 10 days of enrolling)
- Once you get access to the digital ID card, submit a copy to the CHIP Center at daemen.edu/healthupload. Student athletes should also submit a copy to the athletics department through the ARMS software system.

˿Ƶ’s student health insurance plan is through United Healthcare and is compliant with the Affordable Care Act, providing affordable, comprehensive insurance benefits with low prescription costs, telehealth & telecounseling services.

Students participating in the clinical portion of their academic program: Nursing, Physician Assistant, and Physical Therapy are required to have active health insurance that provides comprehensive coverage during the entirety of their clinical experience(s).
